Keep your hands clean

  • Wash your hands often with soap and water for at least 20 seconds (at minimum on entry and exit of a facility)
  • If soap and water are not available, use an alcohol-based hand sanitizer.
  • Avoid touching your eyes, nose and mouth.
  • Cough or sneeze into your elbow.
  • Avoid touching surfaces people touch often.
  • Instead of a handshake, give a friendly wave.

Keep your distance

  • Keep a distance of 2m between coworkers, contractors, visitors and customers.
  • If physical distance cannot be maintained wear a face mask (ASTM Level 2 min).

Wear face coverings

  • Wear a face covering to control possible spread of respiratory droplets.

The Town of Whitchurch-Stouffville continues to monitor and follow advice from public health officials at the federal, provincial and regional levels. The Town is taking a responsible and measured approach to reopening, which is guided by the Government of Ontario’s Roadmap to Reopen and the Town’s COVID Safety Plan (available upon request) – for more information see Stouffville Reopens . The health and safety of our visitors, community and staff remains our number one priority.


Information collected in this form will be used to screen you for COVID-19 risk factors prior to entering a Town facility (as per O.Reg 364/20). Contact information is stored in a secure manner for 30 days as per Public Health requirements.

Ontario’s health privacy legislation, Personal Health Information Protection Act (PHIPA) establishes the rules regarding your personal health information. The Town will not disclose this information to external third parties unless required by law.
