Please follow these fundamental public health measures to help prevent the spread of COVID-19 and follow all site-specific instructions upon arrival. For programs, please also discuss these measures with your child/participant.

 If you are ill – Stay Home!

If you begin to feel ill while in a Township facility, notify a staff member immediately.

 

WEAR YOUR MASK

  • The wearing of non-medical masks/face coverings is mandatory in all indoor Township facilities that are open to the public, as per the directive from York Region’s Medical Officer of Health.

  

WASH YOUR HANDS:

  • Wash your hands often with soap and water for at least 20 seconds
  • If soap and water are not available, use an alcohol-based hand sanitizer (more than 60% alcohol)
  • Use alcohol-based hand sanitizer upon entering and leaving a facility

 

 COVER UP

COver up

  • Cover coughs and sneezes with a tissue or sleeve
  • Use any necessary personal protective equipment, as directed

 

KEEP YOUR DISTANCE

distance

  • Avoid close contact with others by staying more than (2) metres or (6) feet apart from other people
  • Instead of a handshake, give a friendly wave

 

Other Hygiene Considerations

  • Avoid touching your eyes, nose and mouth
  • Avoid touching surfaces people touch often
  • If you are a staff member – ensure surfaces are regularly disinfected in accordance with Township Procedures

The personal information collected through this form is collected under the authority of the Municipal Act, 2001, S.O. 2001, c. 25. The information will be used to screen you (and persons you provide care for) for heightened COVID-19 risk factors prior to entering a Township facility or participating in a Township program. In the event of a confirmed COVID-19 diagnosis that coincides with your visit to a Township facility or participation in a Township program, your name and contact information may be provided to public health authorities for purposes of contact tracing.  Questions about the collection and use of this information should be directed to the Human Resources Division.
