The City will continue to monitor the pandemic situation and all protocols are subject to change based on changing conditions.

Please follow these fundamental public health measures to help prevent the spread of COVID-19; for programs, please also discuss these measures with your child/participant. Please follow all site-specific instructions upon arrival.

If you are ill – Stay Home! If you feel ill during the day, notify staff immediately.

Keep your distance:

  • Always keep a minimum distance of 2 metres between yourself and others (including staff and other program participants).

Hygiene Considerations:

  • Wash your hands often with soap and water for at least 20 seconds
  • If soap and water are not available, use an alcohol-based hand sanitizer (more than 60% alcohol)
  • Use alcohol-based hand sanitizer upon entering a facility
  • Avoid touching your eyes, nose and mouth.
  • Cover coughs and sneezes with a tissue or sleeve
  • Masks/face-coverings are mandatory indoors for City facilities, in accordance with City of Burlington By-law 62-2020 as amended by By-law 64-2020
  • Avoid touching surfaces people touch often
  • If you are a staff member – ensure surfaces are regularly disinfected in accordance with City guidelines

The personal information collected through this form is collected under the authority of the Municipal Act, 2001, S.O. 2001, c. 25, the Reopening Ontario Act, 2020, S.O. 2020, c. 17, and the Occupational Health and Safety Act, R.S.O. 1990, c. O.1. The information will be used to screen you (and persons you provide care for) for heightened COVID-19 risk factors prior to entering a City facility or participating in a City program. In the event of a confirmed COVID-19 diagnosis that coincides with your visit to a City facility or participation in a City program, your name and contact information may be used by the City or provided to public health authorities for purposes of contact tracing. If you are an employee, your information will be used by Human Resources and (shared with) Apex Occupational Health Solutions Inc., a firm contracted by the City, to assist with administration of the City’s COVID-19 Staff Vaccination Policy, including, but not limited to, verifying compliance.
